I can't find a way to add a flag to a line item. I was a windows user previously and never had an issue but this action is not listed anywhere in the Quicken Mac Subscription. What am I missing?

The only "flag" available in QM2019 is the Status column in which transactions can be marked reviewed or not reviewed. The icons here change automatically depending on manual/downloaded status, but can only be cleared by marking as reviewed.
If that doesn't serve your purpose, some people will use the Action column as a custom indicator. You can put pretty much anything you like in this column for any purpose. You don't say the purpose you desire for flagging, but this might work for you.
Another option is to use Tags. Again, you can define tags however you want. These have the advantage that reports can be customized around them.Quicken Mac Subscription; Quicken Mac user since the early 90s6
