Quicken Randomly losing transactions

Wrong balances consistently over the last week when opening quicken file. Performed 2 balance adjustments so far but next time its off again. Tried pulling an old data file and updating but it is wrong too - by a different amount! Help

For instance, where is your main/active file located (external hard drive, cloud-based storage, local hard drive)? Also, are the accounts affected managed manually or do you download transactions from the bank? 

    Please elaborate so that we may be able to provide the proper guidance.
