investment account cash balance corrupted again

My main IRA account, checking accoungt, and credit carfd account were updated and reconciled through July 31 three days ago.  Since then I have done nothing but enter a few debit transactions in my checking account and a few charges in my credit card account.  Now I notice that my IRA account suddenly has a month end cash balance which rarely happens.  This account contains history since it was created with a 0.00 balance 12/31/2003. 

The account always uses Fidelity Government Cash Reserves for any cash balance, meaning that other than pending settlements there should be no balance.  Suddenly, there is a cash balance which varies by several thousand dollars.   

Reviewing the register for this account,  I see that the cash balance is now incorrect all the way back to December 2004 and there are literally hundreds of

"XIn/XOut Balancing Cash Adjustment" 

transactions in the register between 12/31/2003 and 11/30/2005 that should not be there.  

While the cash balance varies through the months and years, the common amount seems to keep going back to a 2790.42 figure.

Two other active investment accounts with the same financial institution that have not yet been updated for July correctly show a consistent cash balance of 0.00.

Also, now trying to scroll through the register for this account slowly skips along one transaction at a time, while scrolliing through 26 other historical investment accounts works smoothly as it should.

My currentl file is RQ2015 which contains data from 2015 to current and investment data since 1943.  So far I have reloaded 8/6/2022 and 8/5/2022 backups and both are also corrupted in this account.

I do not download transactions.

A hard disk (which is actually an SSD) check shows no errors. 

A Quicken file validation only shows the usual:

Fidelity Government Cash Reserves NEVER has a share price other than $1.00 and NEVER has stock splits.

This account did not even exist until 7/1/2019,  after the dates of the first two 'errors'.  There was a sale on 12/31/2019 of 2613.06 shares at $1.00

Someone please explain this.
