Fidelity 401k transactions incorrectly classified as $0 bond sales

I know there have been a number of issues and fixes related to Fidelity in recent weeks, however today I encountered 2 new ones (to me at least).

Sales of mutual funds in my 401k that are done to automatically pay the quarterly management fees all came through incorrectly as follows:

  1. Out of 6 transactions, 4 were 'unidentified security'. only 2 (both Dodge and Cox funds) identified the correct ticker.
  2. All came through for 'bond sales' for $0 total value. When editing the details, the sale amount was incorrectly classified as sales commissions, so the total value was then 0
  3. All came through to post the cash to a separate account, even though I do not have this set up with a different cash account (ie. they were SOLDX vs SOLD).

So I had to manually edit and correct every transaction.

In addition, 2 dividend payments that usually get (correctly) classified as 'reinvest dividends' were incorrectly classified as a cash payments, so the total shares were out of balance.

I usually never have these transaction issues with Fidelity and I believe these may have been caused by some of the other recent 'fixes' that were rolled out

Anyone else having these issues now?
