In Fidelity brokerage account, old transactions keep getting downloaded again and again.
This started happening after new connection update to "express web connect+"
Is it a known issue? Is there a workaround?

It sounds like you are experiencing duplicate transactions. Please review this Support Article and apply the suggested steps therein.
As stated in the article, given that the duplicates are not a one-time thing, you will need to contact Quicken Support directly.

When I select Fidelity account, I don't see any "Register Columns gear icon at the top right of the account register".

Investment account registers don't have the same options as other account types, so that step would not apply with an investment account. I apologize for not clarifying that earlier.
