I try to change a password in Vault but Add, Delete, and Change Password buttons are all grayed out. In the past, I have not had issues updating passwords here. Suggestions?

If the Set Up New Password Vault option is available, it typically indicates that no Password Vault is currently associated with this data file. In that case, you should be able to create a new Password Vault by selecting that option and following the prompts.

That is not the issue. On that tab only Set up new Password Vault is grayed out - the other three options are available.

If the options you are referring to are the ones shown in the image below, then what you're seeing is expected behavior. Accounts connected via Express Web Connect or Express Web Connect+ no longer require Quicken to store your password for transaction downloads because these connection methods use a different authentication process.
As a result, if you need to update the credentials associated with one of these accounts, you will typically need to either reset the account or deactivate and reactivate the connection. Doing so will prompt Quicken to reauthenticate with the financial institution using your current login credentials.
