How can I stop Quicken to place placeholder for Merrill Lynch account?

Quicken keeps adding a placeholder for Merrill Lynch account and mistaken by close to million dollar amount.  Also how to remove it?

    About Placeholders

    "N/A" in
    certain fields of an investment transaction indicates that you have
    Placeholder transactions in your account that need to be resolved.

    For more about
    Placeholders please go into Quicken Help (press the F1 key from anywhere in
    Quicken) and perform a Contents search for "placeholder".

    You may also want to review the Price History for each of the securities you own in this account. Somehow an incorrect price quote may have been downloaded and is now skewing results.
