why are accounts being moved to "separate" category on my account list?

To review or fix that, go into Tools / Manage Hidden Accounts.
If you want to move an account out of "Separate" status back to where it should be in the Account Bar, clear the checkmark in the "Keep ... separate" column for this account.

If you are referring to the Account list on the left side, this is because the accounts are closed or you set them not to be included in your net work calculation. Quicken does not delete the account unless you expressly delete them.
