Each question has an "Other (please specify)" text input box. The input box lacks a radio button. So is the intent that we can choose an item with a radio button and add a comment under "Other"? Or does typing into "Other" supersede a radio button choice?

Hello @Rocket J Squirrel
Great question and thank you for reaching out! Yes, the intent is that an item with a radio button can be selected and a comment left if desired.
Adding a comment will not supersede the radio button choice
Hope this helps!
