vault passwords disappeared-now unable to re-add

my vault passwords disappeared when restoring data file to new laptop; now i am unable to add passwords back to vault or do a single account update; I have no idea what happened;

further to my vault issue; when I try to do a single account update and add my password, then hit update; I get a popup telling me to add my password to that same bank
