Bank of America payment was cancelled:
"Reason:
Your payment was canceled because Bank of America requires that you enter a new one-time code verification. Please try your payment again and enter the one-time verification code."
I refreshed the biller and entered the one time verification code before sending the payment.

@elbuffaloniian - is it possible that you inadvertently entered an old code? Just a guess on my part. Sometimes you need to wait a bit before a new code is transmitted. If I am ever in doubt, I always hit "send new code".0
