Placeholder being added in error when updating investment account

I am getting a placeholder added for a security that is not in this account. It is a T Rowe Price single mutual fund account and the placeholder asking to add cost info on for a Vanguard Mutual Fund. When I delete it the placeholder reappears when at next download
