Over the last year or so, I occasionally notice missing transations

I recently noticed a missing transaction and when I checked my reconcilation for last month, it was there. I have had this happen many times over the last year or so, but I re-enter it and coninue on. It is bothersome to me, because I cannot figure out what is causing such a thing. I usually notice when I go to check something and realize my balance is different (more) than it was the day before…
