Contributions to 401K Not working

Background (to try to skip some of the standard responses I find when searching this)

I have my paycheck set up using the paycheck wizard.

I have both an employer and employee contribution section.

Most of the time this works, but sometimes one or the other doesn't get transferred into the 401K account (it is still in the Paycheck though).

Also if when going through the transactions I notice the was a change (for example to the amount of the Employer contribution) if I edit that in the paycheck, the entire deposit in the 401K will go away.
