Duplicating transactions

I was working in Quicken when a Windows screen opened up, a list of commands executed and the window closed. It looked like an update to Quicken while it was open. Afterward, there were 29 random transactions duplicated involving six different accounts spread over five years. The only difference between the new transaction and the original transaction were the Reconciled check on the original versus nothing on the new. Easy to find and delete with this, but why did it happen.

Have you recently changed the download method (say, Direct Connect to Express Web Connect, or another change)? Or have you recently disconnected and re-connected those accounts?

Any other (excepting transactions) changes to those accounts?

No changes that I know of. A new account was added to one of the institutions involved. But I was not doing any type of downloading at the time this happened. I was doing a reconciliation of an account not involvedin the duplication.
