missing transactions

I often encounter this problems but it has now gotten worse. There are missing transactions in accounts and the balance does not match up. In a couple of the accounts, I had to go back 5 years and input the missing transactions so that the balances agree. What do I do to fix this time consuming problem.

In order to better assist you, I have a few questions:
- Which financial institutions are affected?
- Are all of the missing transactions within the same date range, or do they vary by account?
- Have you noticed if the missing transactions are specific to a certain type—like deposits, payments, or transfers?
- Have you recently restored from a backup or made any major changes to your data file?
- When you re-entered the transactions manually, did future downloads duplicate them, or did they remain missing?
