401k Account Transactions cannot be made without a "Balancing Cash Adjustment" appearing.

If I Enter a Transaction, Q adds a Balancing Cash Adjustment. Doesn't matter if it's a Buy or Sell Transaction. If I delete the BCA, the corresponding transactions are deleted by Q.
Also, my Holdings are completely fouled up, effective 12/28/19.
If I show the holdings "As of" 12/27/19, they're correct and only show the funds currently invested in. If I change the "As of" date to 12/28/19 or after, the holdings show funds that go back 20yrs!
I have read about Placeholders and all the recommended actions with regards Placeholders - nothing changes, nothing works.
