Categories not showing sub-categories correctly

Question

When looking at a sub-category with a parent category in the main category view, it does not show the parent category correctly. When I try to edit the category to add the parent category, it no longer shows the parent in the list to add.

I have run Validate file and Super Validate file and it was no help. I got the errors below, but was not able to move the categories into the correct groups either.
