What is the format of the CSV file for a security's Price History?

I need to construct a .csv file with historical prices to add them to a security. The problem is I don't know what the format of that file should look like. I'm assuming it should start off with the security name or symbol then have a list of dates and prices.
Does anyone have an example of a .csv file that shows the format?
