Securities Mismatch in 401k account

What's the best way to reconcile share discrepancies? I'm seeing a huge mismatch between my Fidelity and Quicken accounts. I've tried resetting the account and even deleting and re-adding it from scratch, but the discrepancies persist. I suspect it might be an issue on Fidelity's end, possibly related to rollovers in my 401k. Any advice or suggestions would be greatly appreciated.

What items are you using to identify this mismatch?
Can you use your Fidelity statements to work backwards and see if you can identify WHEN the mismatch happened?
