Security Mismatch Popup

I have 4 IRA accounts among all of the investing accounts which I track in Quicken that get security mismatch popups when I download transactions. My wife and I share the use of the data file in which these accounts are tracked. When she does a download, she does not get the security mismatch popup. The title for the error message I get for two of the accounts reads "[Security Symbol].popup [Name of the Security] ([Security Symbol]) ([Security Symbol])." The other two accounts get a slightly different title: "[Name of the Security] ([Security Symbol]) ([CUSIP ID of the Security]). My question is how do I eliminate getting these mismatches?

And, what financial institution holds these accounts?

In your 1st graphic, how did Disney get a CUSIP of DIS (which isn't a valid CUSIP)? Likewise for WalMart?
