Quicken used to reconcile manually entered transactions with the bank download. But recently, I have noticed that it no longer does that, so I have to go in and delete duplicate entries. What gives with that?

That should be the result of you selecting "Reconcile using online balance" in the "Online Services" tab of "Account Details."

Which suggests that selection of "Reconcile using online balance" somehow got changed.

One thing should not have to do with the other. "Duplicate entries" should not occur, period, no matter how you reconcile an Account.
What financial institution is involved here and what downloading method are you using?
Are you allowing Quicken to automatically enter downloaded transactions into the account registers? That can sometimes be problematic because it can result in downloaded transactions not getting matched by Quicken to the manually entered transactions. When this happens it will result in there being duplicate transactions in register so only one of them will be included in the reconciliation with the other one (likely the manually entered one) not being included.
Go to Edit > Preferences > Download transactions and if the box is checked for Automatically add to banking registers uncheck it followed by OK. Then you will get the opportunity to view downloaded transactions so you can ensure sure they are properly matched to the manual transactions before the transactions are entered into the register rather than entering them as unmatched duplicates.
