Attachment Paperclip

If I add the attachment 2 times the paperclip show up. sometimes it works and shows up other times it doesn't. I very much need this to work right.

    When did this issue begin to occur? After a recent update? Do you receive any error codes or messages when attempting to add the attachment the first time?
