out of context prompt when creating a transaction

Sometimes when creating a transaction I get a prompt something like: are you sure you want to change a reconciled transaction?
If I say no the transaction is not entered, if I say yes the transaction is entered.
The prompt is totally outt of context. Why is this happening?

Does this happen when you're creating a transaction in a previously blank line in the account? Or when you've copied a transaction and are changing it? Or when?
Also, does this happen in all account types? Or only some?
