How to connect to Nationwide to add annuity?

I have a Nationwide New Heights Select9 Fixed Indexed Annuity. I have tried some of the Nationwide connections shown on the Add Accounts screen, but they seem not to work. Can anyone help?

Which instances are you attempting to connect to (see screenshot below)? What exactly is occurring when attempting to add your account? When did this issue begin to occur? Are you receiving any error codes or messages?
